GREAT NEWS – UPDATED PRACTICE TRANSITION NOTICE

Dear Patients,

We are pleased to share an encouraging update regarding the future of our practice. 

We previously notified patients that Dr. Gabriel Rodriguez anticipated retiring from active clinical practice and that the Practice would begin an orderly closure. At that time, this was the only responsible transition plan available. 

Since that announcement, a potential continuity-of-care opportunity has developed that may allow the Practice, our established clinical team, and patient services to continue under new physician ownership and medical leadership. The proposed transaction and related clinical arrangements are still being finalized and have not yet closed. 

During this evaluation and transition period, the Practice will remain open and will continue caring for established patients. Our goal is to preserve continuity, retain our experienced physician assistant and staff whenever possible, and minimize disruption to established treatment plans, subject to each patient’s individual clinical needs and the independent medical judgment of the treating providers. 

All customary clinical safety and medication-management protocols remain in effect. Controlled-substance treatment will continue to require individualized clinical assessment, medication reconciliation, review of the Texas Prescription Monitoring Program when required or clinically appropriate, urine drug screening when clinically indicated or required by the patient’s treatment protocol, and complete documentation. 

Dr. Rodriguez’s future clinical role after the transition has not been determined. He may remain available for limited transition assistance, consulting, locums work, or future clinical services, but no commitment has been made. 

Every patient remains free to continue receiving care through the Practice during the transition or to select another physician or medical practice. Patients may request copies of their medical records or authorize transfer of records at any time.

Patients are being instructed that complete medical records and formal records transfers must be requested through VitalCharts / Vital Records Control, the records company handling medical records requests for the Practice.
