Physician Retirement

Dear Patients,

After many years of serving our community, Gabriel A. Rodriguez, M.D. will be retiring from active clinical practice.

The final clinic day and final day for prescriptions will be: Friday, August 7, 2026

The office will remain administratively available through August 14, 2026 to assist with transition-related matters, including medical records instructions and limited continuity-of-care coordination.

Patients are encouraged to establish care with another physician or appropriate medical provider as soon as possible. Patients may contact their health insurance carrier for a current list of in-network pain management physicians or appropriate specialists. Patients may also contact their primary care physician or referring physician for assistance with ongoing care.

Medical Records

Patients may request copies of their medical records or authorize transfer of records to another physician or medical practice. Complete medical records and formal transfer requests must be requested through VitalCharts.

Medication management, including controlled medications when applicable, will be reviewed individually. When clinically appropriate, a reasonable transition bridge prescription, often up to thirty (30) days, may be provided to allow time for the patient to establish care with another qualified provider. Prescription decisions remain subject to clinical judgment, patient safety considerations, pharmacy requirements, and applicable state and federal laws.
